Your comfort and dignity is our top priority and our experienced support workers will make sure you’re always treated with kindness and respect.

Our personal care services include:

In home nursing services

MHCC Care is a Home Care provider of high quality, reliable community registered nursing services. Our qualified Division 1 Nurses come to your home when it suits you to provide the nursing support you need. Our nurses place an emphasis on maintaining your dignity and independence, while supporting your ongoing health needs.Services are provided through private services funding or Home Care Packages.

We can also assist you by:

  • Helping you manage your medicines – we can explain what they do, how they work and provide assistance with administration of medication where required.
  • Providing wound care.
  • Completing a comprehensive assessment of your needs.
  • Referring you to a range of community services where appropriate.

In-home respite

MHCC is able to provide respite care in the comfort and familiarity of the client’s own home through our Care at Home service.In home respite can be provided for a few hours or for an extended period. In home respite can enable a carer to have a break, attend appointments, or rest and recuperate after an illness or injury.

What kind of support is available for the client?

We are able to support personal care needs and assistance with the tasks of daily living, such as showering, dressing, shopping and meal preparation. We can also provide social companionship support either in the home or out in the community.

Our qualified nurses are also available to provide nursing care if required, including medication assistance, wound care and continence support.

Who provides respite care?

Respite care is provided by one of our experienced Care at Home staff, who will come to the client’s home and provide safe, sensitive, private and dignified care.

Our professional and qualified personal carers all hold a minimum Certificate III in Aged and/or Community Care. They also hold a Level 2 First Aid Certificate and have been carefully screened for your safety and peace of mind, including police checks and reference checks.

We also have a team of highly-qualified Division I Nurses available.

How do I book in home respite and what are the costs?

If you would like to book in home respite, please contact our Care at Home team on, or send us an enquiry using the form.

The cost of respite varies depending on whether the client has government aged care funding such as Home Care Packages or Veteran’s Home Care, and what level of package they have.

Alternately, in home respite can be funded privately; no Aged Care Assessment Record is required to access this service privately.

 

Other services

MHCC provides a broad range of allied health and specialised health and wellbeing services within the community to help you regain or strengthen your mobility, independence and general sense of wellbeing.

Case Management

Case Management is the coordination of home and community services to ensure our clients care and support needs are met on an ongoing basis. Specialised staff assess each client’s needs and tailor a plan to suit our client’s care plans.

Home Care Packages

What is a Home Care Package?

A Home Care Package is individual funding (a subsidy) provided by the Australian Government to assist an older person to live in his/her own home with support. There are four different levels of packages and associated funding available and a person is allocated a package depending on his/her needs as assessed by a the Aged Care Assessment Service (ACAS).

What does a Home Care Package pay for?

A Home Care Package is designed to be flexible and offers a broad range of services so you can choose the services that would benefit you most. Some people need help with domestic tasks such as laundry or shopping, others need transport to appointments, or personal care such as assistance with showering or dressing or purchasing aides or equipment. MHCC is a registered and approved provider of Home Care Packages and have coordinated packages for more than ten years for clients across Melbourne.

At MHCC you will be allocated a dedicated Care Advisor who will work with you to determine your needs and preferences, establish a care plan, and ensure you are getting the most value from your package.

How much funding does a Home Care Package recipient receive?
  • Level 1 is $8,785.55 per year or $168.90 per week
  • Level 2 is $15,457.75 per year or $297.20 per week
  • Level 3 is $33,638.40 per year or $646.80 per week
  • Level 4 is $50,990.50 per year or $980.50 per week

How much will I have to pay for a Home Care Package?

Basic Fee

The maximum basic daily fee a provider can charge for a Home Care Package, is up to $10.10* per person *current as of April 2017, if this fee is updated it will be available here.

Income-tested care fee

An income-tested care fee is calculated by the Centrelink, and applies if your income is over a certain amount. Contact Centrelink.

How do I choose MHCC as my Home Care Package provider?

If you already have a package, you can call us on and we will help you with this process.

If you are still waiting for a package, once you are approved, you can ask My Aged Care for it to be provided through MHCC.

How do I get a Home Care Package?

Contact My Aged Care on 1800 200 422 or complete a form online so they can start the process for you. You, or a friend/family member/medical professional can request this for the care recipient. There is no cost for you to register with My Aged Care, or to obtain an assessment.

  • My Aged Care will do a primary assessment with you over the phone.
  • If your needs are low they may arrange access to Commonwealth Home Support Programme, which offers basic assistance such as shopping, personal care and cleaning.
  • If your needs are more complex, they will refer you for a comprehensive assessment with the Aged Care Assessment Team (ACAT).

The ACAT team member will:

  • Ask you questions about your day-to-day living activities, what you need help with and talk to you about your general state of health and specific health conditions. This will help them work out how much and what type of help you need.
  • Talk to you about whether you need more support so you can continue living in your own home or if you might be better supported in an aged care home.
  • Give you information about all of the services that may be available in your local area.

The ACAT will give you approval for an appropriate level Home Care Package.

You will go onto a waiting list with My Aged Care for one of these packages to become available.

You can take advantage of a lower level package and waitlist until your appropriate level becomes available.

How do I get my funding increased if my needs have changed?

Please get in touch with My Aged Care, they will arrange another ACAT assessment to reassess your needs. Please be aware though that a waitlist may apply for a higher level package.
